Recovery Navigators

Contract: Permanent

Full time based at the Greenway Centre, the Petherton Resource Centre and Speedwell.  There is also a full time temporary position at the Speedwell Centre.

Salary: £21,301- £23,284 (pro rata for part time)

                   

Second Step leads a partnership of voluntary sector organisations which deliver community mental health services in Bristol with Avon and Wiltshire Partnership Trust (AWP).  This is carried out via three mental teams: Assessment and Recovery, Crisis and Early Intervention.  All the teams came under the banner of Bristol Mental Health.

 

The successful applicant would receive a comprehensive induction and be fully supported by the multidisciplinary team (MDT) to hold a caseload of low and medium risk service users with severe and enduring mental health needs. You would receive management and clinical supervision and take part in weekly meetings with the MDT with the opportunity for reflection and review of service users on your caseload.

 

As care co-ordinator you would work closely with service users and their carers in the community to enable and assist them to meet daily health, social care and well being needs, in line with individual personal recovery goals. You will collaboratively develop recovery orientated care plans with service users and contribute to ongoing assessment and review of recovery goals.

 

The ideal candidate will understand  the support needs of service users over the age of 50 years old, in a community setting.  In addition candidates need to be aware of the impact of deteriorating physical health on client’s mental health as well as the impact of isolation, with a view to widening their social networks within the community.

 

You will be an excellent communicator, able to build therapeutic relationships with service users and enable engagement with voluntary and statutory organisations across Bristol.

     

The team work on a shift basis within the operational hours of Monday to Friday 8am – 8pm and Saturday or Sunday 9am-5pm. The majority of week day hours  are 9.00am to 5.00pm.

 

The jobs cover bank holiday working and weekends on a rota basis.

 

Please note you will ideally hold a current driving license, either a car or motorbike, and have access to a vehicle.
