Southern Brooks is looking for a Social Prescribing Link Worker to be based in Southmead Hospital, and the contract comes with an annual remuneration of £25k pro rata based on 37 hours per week. The job is offered on a fixed-term contract basis, until August 2023.

The purpose of this Social Prescribing Link Worker role is to support patients who are leaving hospital and assist them in settling back into daily life.

In your first few weeks in this Link Worker role, you can expect to:

• Provide personalised support using motivational interviewing and other techniques to establish the needs of the individual
• Support individuals to address barriers to accessing services or maintaining independence
• Empower individuals to maximise the control they have over their lives
• Co-produce action plans and facilitate follow-through.
