Within this new role you will
- visit people in their homes, get to know them and what is important to them
- provide personalised information and support to people and signpost or refer them to appropriate activities and services
- attend services and activities with the people you support when appropriate
- develop, build and maintain relationships and knowledge of local voluntary sector and community based groups and services.
As a member of the Community Navigator team you will receive full training and support, develop new skills and be part of a new and innovative project to connect people with their communities - making a difference to the lives of older people in North Bristol.
