The Golden Key Service Coordinator (Hospital Liaison) is a new role to support a pilot project to reduce the impact of winter pressures on A&E and hospital services within Bristol. The role will be working closely with hospital teams to identify suitable referrals to the project, with the aim of reducing the length of hospital stay or initial admission for service users. Being a pilot, the role is subject to review, evaluation and change. This job description does not therefore describe a comprehensive list of duties, but a broader range of accountabilities and performance indicators.
