Hours: 37 per week
Salary: £22,779 - £24,899 per annum

We have an exciting and rare opportunity to join Second Step’s Health Link Team who work in partnership with BrisDoc’s Homeless Health Service,
About the role:
The focus of the role is to provide support to people 20-25 people in a variety of settings. Including at the medical centre GP surgeries within the homeless pathway accommodation and within peoples own homes, plus many other settings. Many people are at high-risk and use A&E repeatedly. We support people who are homeless, at risk of becoming homeless, and support to people within the Bristol Homeless Pathways
People on our caseload have significant physical and enduring health problems and/or who struggle to engage with health services. Maybe occasional evening work will be required.
You will have:
We are looking for a person who have minimum 2 years’ significant experience in relationship building, ability to advocate and hold and manage a diverse and highly complex Homelessness focused support orientated caseload
Highly developed multi agency liaison with a “health” focus.
A great sense of humor and ability to work within a very motivated and dynamic Health Team
Whilst this role is based in Central Bristol, you will be expected to travel within Bristol therefore access to transport is desirable but not essential
