We are recruiting a Mental Health Support Worker to join our Primary Navigation Team in South Gloucestershire. This is a community-based service covering Emersons Green, Downend, Staple Hill, Kingswood and surrounding areas.  Internally you will be known as a Recovery Navigator.  Working collaboratively with the client, GP, Social Prescribing Link Workers and other healthcare professionals, you will support clients with moderate to high enduring mental health difficulties to find coping strategies personally and within their community.

building a trusting and collaborative relationship with the client; demonstrating hope and courage

completing individual assessments that will inform the client’s recovery plan.

supporting the client to utilise self-management tools e.g. Wellness Recovery Action Plan to stay well.

using a range of interventions, approaches and tools (motivational interviewing, active listening, navigation, solution-focused support) that enables people to move positively through their recovery.

navigating the ‘wider system’ which includes supporting the client to access appropriate support.

enabling clients to make informed decisions about their mental wellbeing, maximising their independence and providing advocacy role where appropriate.

About you

You will:

be passionate about improving the lived experience and health of people using mental health services

be confident in working independently within the local community.

have a minimum of 2 years proven experience of working with people with mental health needs, or people with complex needs

be able to manage a caseload of 16-20 at any one time.

proven experience of carrying out needs and/or risk assessments and the support planning process

experience of working with a number of individuals with competing needs and priorities
